Electric muscle stimulation to preserve muscle mass and performance: from space to earth

Collaboration to translate knowledge from space medicine into clinical treatment in the field of electric muscle stimulation

The works at the project will be divided in 5 stages. At the Stage 1 (“Development of the study program and performance of pilot studies”, autumn – 29 Dec 2017) the Russian side will develop the detailed study protocol for a randomized controlled trial on the use of different EMS stimulation protocols to prevent sarcopenia under conditions of onground models of weightlessness, and in elderly people; prepare of the research setting, including testing of methods of cardio-vascular and sensory-motor systems’ state assessment; recruit the study subjects, perform the pilot studies, perform the patent studies, submit the research protocol to the Ethical Committee; perform at least one event directed to demonstration and popularization of the project; prepare at least one publication. The Swiss side of the project will develop the detailed study protocol for a randomized controlled trial on the use of different EMS stimulation protocols to prevent sarcopenia in patients with heart failure, stroke patients and elderly people; prepare of the research setting. including testing of methods of cardio-vascular and sensory-motor systems’ state assessment. At the Stage 2 (“Laboratory and medical studies”, 01 Jan – 31 December 2018) the Russian side will perform the studies of efficacy of low-frequency EMS in counteracting of muscle detraining caused by effects of onground space flight factors simulation; define the efficacy of different duration courses of EMS in counteracting of sarcopenia in heart failure patients; provide the preliminary data processing; perform at least one event directed to demonstration and popularization of the project. The Swiss side will perform the testing of developed study protocols in heart failure patients, elderly people and patients after stroke; develop the medical-technical requirements to a special easy wearable fabric for EMS to facilitate widespread use. At the Stage 3 (“Continuation of laboratory and medical studies”, 01 Jan– 31 Dec 2018) the Russian side will perform the studies of efficacy of high-frequency EMS in counteracting of muscle detraining caused by effects of onground space flight factors simulation; study the efficacy of low-frequency EMS in counteracting of sarcopenia in heart failure patients with the choice of optimal amplitude and duration of stimulation as well as duration of EMS courses; perform at least one event directed to demonstration and popularization of the project, prepare at least 2 publication. The Swiss side will continue the studies of EMS effects in elderly people and patients after stroke; continue developing the medical-technical requirements to a special easy wearable fabric for EMS to facilitate widespread use; take part in the event directed to demonstration and popularization of the project and preparation of publications. At the Stage 4 (“Accomplishment of laboratory and medical studies. Data analysis, 01 Jan – 31 December 2019”) the Russian side will perform the analysis of the studies’ results comparing the effects of low- and high frequency EMS in countermeasure of sarcopenia caused by simulated space flight with the choice of optimal EMS type for maintenance of different muscle properties (muscle strength, muscle endurance, etc.); perform the additional patent studies; continue medical studies of EMS effects in elderly people, data analysis of these studies. The Swiss side will continue the studies of EMS effects in elderly people and patients with heart failure and after stroke, perform the data analysis; further evaluation of special fabric wearable with electrodes which can be easily applied by elderly people and patients at home; perform at least one the event directed to demonstration and popularization of the projects’ results. At the Stage 5 (“The formation of the final product”, 01 January 2020 – 31 July 2021 ) the Russian side will perform the analysis of the efficacy of developed protocols of EMS in elderly people/heart failure patients; develop the medical-technical requirements for multifunctional electromyostimulator directed for use in clinics, by elderly people and by heart failure and stroke patients themselves; make patent application; perform at least one event directed to demonstration and popularization of the project, prepare at least 2 publication. The Swiss side will finish the analysis of the studies of EMS effects in elderly people and patients after stroke; take part in the development of the medical-technical requirements for multifunctional electromyostimulator directed for use in clinics, by elderly people and by heart failure and stroke patients themselves; make patent application.
